Every night guard case opens with a material selection conversation. Hard acrylic (1.5 mm to 3 mm occlusal thickness) handles heavy bruxers and parafunctional clenchers. Dual-laminate, hard on the outside and soft on the inner surface, suits patients who have rejected the all-hard appliance for comfort reasons. Soft single-layer is reserved for light grinding or short-term athletic bite protection. The prescription form asks three questions: arch, material preference, and whether a digital scan or PVS impression is coming. That up-front triage eliminates the back-and-forth that adds days to a case. For digital submissions, intraoral scan files in STL or OBJ format feed directly into our 3Shape Dental System workflow. A technician reviews the occlusal relationship and flags any scan artifacts before the milling file is released. Milled hard acrylic blanks are cut on a 5-axis unit, then hand-finished to remove mill marks from the tissue surface. No lab-side occlusal adjustment is made without a written request from the prescribing doctor, because we do not correct what we cannot verify chairside. PVS impressions are poured in Type IV die stone within four hours of receipt. The resulting model is scanned and enters the same digital fabrication path as a direct intraoral scan. This hybrid approach means a practice still running analog impressions for appliances does not receive a different quality tier than a fully digital practice. The output is the same milled appliance from the same blank. Finishing follows a defined sequence: trim on the model, polish occlusal surface to high shine, polish tissue surface to smooth satin, inspect edge thickness with a digital caliper at four reference points, and log the measurements before packaging. Any appliance that misses the 0.1 mm tolerance threshold at any reference point is remade before it ships. The caliper log travels with the case file, not a separate spreadsheet, so the technician who made the appliance owns the QA record. Shipping uses a rigid appliance case, not a foam-lined mailer, and the packing slip includes the direct extension of the technician on the case. If the appliance does not seat at delivery, that technician is the first call, not a general customer service queue. Typical resolution on a fit question is same-day guidance and, when a remake is warranted, a 3-business-day expedited turnaround at no additional charge.
