<p>The preparation decision tracks three variables: how much sound tooth structure remains, how much occlusal load the restoration will see, and whether you can achieve the isolation needed for adhesive bonding. Get those three right and the ceramic system follows.</p><p>Choose a minimum prep crown when the patient is under 40, the tooth is vital with intact enamel circumferentially, the case is anterior or premolar with light-to-moderate occlusion, and you can isolate cleanly for adhesive cementation. Pick a full prep crown when the tooth is endodontically treated, when the patient is a documented bruxer, when you need monolithic zirconia for posterior molars under heavy load, or when existing restorations have already compromised more than 30% of the coronal structure. Go with a full prep when subgingival caries or fracture lines require margin extension below the CEJ.</p><p>The gray zone is the vital posterior premolar with a moderate MOD restoration. Either approach can work; the decision often comes down to whether the remaining enamel ring is intact and whether the operatory can achieve rubber dam isolation at cementation.</p>
