Pick delayed loading when the site is grafted, the bone is Type III or IV, the patient bruxes, or systemic factors like uncontrolled diabetes or heavy smoking are in play. For full-arch All-on-X, go with immediate loading when cross-arch splinting can distribute load across four or more implants achieving cumulative stability targets; opt for delayed staging when even one implant falls short on torque.</p><p>The prosthetic side has to be ready before the surgical decision is final.
