Where each one earns its place

<p>The prescription decision usually comes down to three variables: where the tooth sits in the arch, how the patient loads it, and what the patient sees in the mirror. For most 2026 single-unit cases, all-ceramic is the better answer, but PFM is not obsolete and we will not pretend otherwise.</p><p><strong>Pick lithium disilicate when</strong> the case is an anterior single unit and esthetics drive the prescription, especially with adjacent natural dentition where light transmission matters. <strong>Choose monolithic zirconia when</strong> the case is a posterior molar in a confirmed bruxer or heavy-occlusion patient where chip risk would kill a layered restoration. <strong>Go with PFM when</strong> the case is a 4-plus unit posterior bridge where the metal substructure earns its keep, or when a long-time patient specifically requests the restoration they have trusted for 30 years.</p><p>Our lead ceramist Stoyan Dobrikov reviews every anterior case before milling. If the prep, shade map, or photography is not sufficient for the prescription, we call the office the same day rather than guess. That call is the difference between a seated case and a remake.</p>

Our Ceramics department leans heavily into lithium disilicate and monolithic zirconia because that is where the case volume has shifted: roughly 80% of our crown-and-bridge work in 2026 is all-ceramic, with PFM holding steady on long-span bridges and a small set of legacy-preference cases. Our lab-wide remake rate sits under 2.5%, and every ceramic case ships with the technician's direct line on the case slip.</p><p>If your prescription pattern is still primarily PFM and the cases are performing for your patients, we are not here to talk you out of it. PFM done well outlasts all-ceramic done poorly. What we will say: if you are sending anterior single units to a lab that does not call you back when the shade map is ambiguous, the restoration is going to come back. That is the part we fix, regardless of material.</p>
