The clinical problem an angled abutment solves

Implant bodies follow bone, not prosthetics. When the available ridge forces an implant 15 to 30 degrees off the ideal restorative axis, a straight abutment puts the screw access channel through the facial of an anterior crown or through the occlusal cusp of a posterior. An angled abutment redirects that emergence to a restorable position. Without it, the case becomes a cement-retained workaround or a remake.

When should I use a custom angled abutment instead of a stock one?
Choose custom when the case is in the esthetic zone, when soft-tissue biotype is thin, when emergence profile matters for the final restoration contour, or when the angulation correction needed exceeds the 15 to 30 degree stock range. Custom also makes sense when the gingival margin needs to sit at a specific depth relative to the zenith of the adjacent teeth. Stock is fine for posterior cases with thick keratinized tissue and standard angulation needs.
Can angled abutments be used with screw-retained crowns?
Yes. The whole point of the angle correction is to bring the screw access channel into a restorable position so a screw-retained crown becomes viable. On a 25 to 30 degree off-axis implant, an angled abutment can move the access channel from the facial of an anterior to the cingulum, or from the cusp tip of a posterior to the central fossa. This is preferable to cement-retained on most implant cases because retrievability is preserved.
What information does the lab need to design a custom angled abutment?
An intraoral scan of the implant position with scan body in place, a scan of the opposing arch, a bite registration, and ideally a CBCT for the surgical context. A photo of the soft-tissue contour helps for esthetic zone cases. If the referring clinician has a planned restoration design (digital wax-up or temporary), send that too.
