How is an implant abutment selected?

For general dentists and prosthodontists, the stock-versus-custom decision usually starts with the implant's emergence angle. Implants placed within roughly 15 degrees of the prosthetic axis can accept a stock abutment without significant compromise. Beyond that threshold, a custom-milled abutment is the reliable path. Tissue depth is the second gate: deeper subgingival margins in the anterior zone demand a custom contour to support soft-tissue architecture and prevent a gray hue at the gingival margin.

Oral surgeons and periodontists deal with an additional variable: the emergence profile through soft tissue. A custom abutment designed from the actual CBCT and intraoral scan data lets the lab engineer the exact convexity that supports papilla fill and avoids tissue blanching. Generic stock components cannot replicate that geometry.
