Are dental ceramics biocompatible?

Yes. Dental ceramics, including feldspathic porcelain, lithium disilicate, and zirconia, are widely accepted as biocompatible materials. Zirconia in particular has been studied extensively since the 1990s and shows no measurable cytotoxicity, no metal-ion leaching, and no significant inflammatory tissue response. The ISO 10993 standard governs biocompatibility testing for all ceramic restorations before clinical use.

For prosthodontists and general dentists selecting a ceramic for anterior or posterior restorations, biocompatibility is the baseline, not the differentiator. All three major ceramic families in current clinical use, feldspathic, lithium disilicate (e.g., IPS e.max), and zirconia, have cleared ISO 10993 biological evaluation. What varies is mechanical behavior: flexural strength for zirconia runs 900-1200 MPa versus 350-400 MPa for lithium disilicate, which directly affects case selection on posterior implant-supported crowns.
Zirconia's biocompatibility profile has accumulated the most peer-reviewed volume since Christel et al. documented its osseointegration compatibility in 1989. Soft tissue studies through the 2010s confirm minimal plaque adhesion and a gingival response comparable to gold. For periodontists managing emergence profiles on custom abutments, that surface behavior matters at the tissue interface, not just in the laboratory stress tests.
The clinical question referring dentists actually face is not whether ceramics are biocompatible, they are, but which ceramic fits the case biomechanically and esthetically. Shade depth, translucency grade, and layering protocol vary significantly between full-contour monolithic zirconia and pressed lithium disilicate.
