How bonding agents work

A bonding agent does two jobs at once. Mechanically, it flows into the microscopic porosities created when phosphoric acid etches enamel and dentin, then cures into resin tags that lock the restoration to tooth structure. Chemically, functional monomers like 10-MDP form ionic bonds with calcium in hydroxyapatite and with metal oxides on the surface of zirconia and non-precious alloys. The 2023 Journal of Adhesive Dentistry meta-analysis on universal adhesives reported mean dentin bond strengths of 35-45 MPa across the major systems in current clinical use.

Do I need a separate primer with a universal adhesive?
For direct composite to enamel and dentin, no. A universal adhesive with 10-MDP is single-bottle by design. For indirect restorations the answer is yes, you still need substrate-specific surface treatment before the adhesive. Lithium disilicate requires hydrofluoric acid etch plus silane. Zirconia requires air abrasion plus an MDP primer if the universal adhesive does not already carry MDP at sufficient concentration. What is the difference between etch-and-rinse and self-etch bonding?
Etch-and-rinse uses 32-37% phosphoric acid as a separate step to demineralize enamel and dentin before the adhesive goes down. Self-etch adhesives carry acidic monomers in the bottle, so etching and priming happen in one step without a rinse. Etch-and-rinse produces higher enamel bond strengths; self-etch reduces post-operative sensitivity on dentin. Universal adhesives let the clinician choose either protocol per substrate, which is why they have displaced the older generations in most restorative practices since 2015.
How long does a bonded restoration last?
Clinical survival depends on substrate, isolation, and bonding protocol more than on the bonding agent brand. Bonded lithium disilicate inlays and onlays show 95%+ survival at 10 years in the peer-reviewed literature when bonded with a silane plus adhesive resin cement protocol under rubber dam isolation. Direct composite restorations bonded with current universal adhesives show 90%+ five-year survival in posterior teeth. Debond failures cluster around isolation breakdown and skipped silane or MDP primer steps, not around the adhesive itself.
