What is a custom implant abutment?

Custom abutments are indicated when the implant angulation, soft tissue depth, or emergence profile cannot be adequately supported by a stock component. Anterior sites, deeply placed implants, tissue-level esthetic zones, and off-axis posterior placements typically benefit. Stock abutments remain viable for well-positioned posterior cases with shallow, symmetric tissue. Send the case with a scan body or implant-level impression so the lab can evaluate site-specific needs.

Custom abutments are commonly milled from titanium for strength in posterior or high-load cases, or from zirconia bonded to a titanium base for esthetic anterior cases where tissue show-through is a concern. Hybrid options combine a titanium interface with a zirconia superstructure. Material choice depends on the implant system, load, tissue biotype, and whether the final restoration is screw-retained or cement-retained.

Submit a full-arch digital scan or PVS impression with an implant-level transfer or scan body specific to the implant system and platform. Include the opposing arch, bite registration, shade for the final restoration, and any soft tissue reference. Confirm implant manufacturer, diameter, and platform. A photo of the tissue emergence at scan time is helpful when the final restoration is esthetic.

Yes. The lab can design custom abutments for either retention type based on your prescription. Screw-retained designs integrate the abutment and crown or place the access channel through the restoration when angulation allows. Cement-retained designs position the margin at or slightly subgingival to protect tissue and simplify cement cleanup. Indicate your preference and any access channel angulation limits on the Rx.

The emergence profile shapes how soft tissue is supported from the implant platform to the restorative margin. A properly contoured custom abutment maintains papilla height, prevents blanching, and creates a natural transition to the crown. Overcontouring can cause recession and inflammation, while undercontouring leaves black triangles. The lab designs the profile based on your scan of the healed tissue or provisional-guided contours.
