How long does a zirconia crown last?

A properly fabricated monolithic zirconia crown lasts 15 to 20 years in typical clinical use, and cases with clean margins, correct occlusion, and appropriate cement selection routinely run past that window. The failure curve on zirconia is not shaped like the ceramic wearing out. It is shaped like a bathtub, most failures cluster in the first 24 months (fit, cement, or occlusal issues) or after year 15 (recurrent decay at the margin, not fracture of the crown). If you are asking this question because a patient's crown failed at year four, the ceramic is almost certainly not the reason.

Is zirconia stronger than lithium disilicate for posterior crowns?
For posterior single units under heavy occlusal load, monolithic 3Y-TZP zirconia (around 1200 MPa flexural strength) outperforms lithium disilicate (around 400 MPa) on fracture resistance and long-term survival. Lithium disilicate holds an esthetic advantage in the anterior and works well in low-load posterior cases, but for second molars, bruxers, or any case where load is the primary concern, monolithic zirconia is the more defensible choice.
Do zirconia crowns wear down opposing teeth?
Polished monolithic zirconia causes less antagonist enamel wear than glazed zirconia or feldspathic porcelain. The wear risk is real when the occlusal surface is left rough after adjustment. Every zirconia crown that gets chairside-adjusted must be re-polished with a diamond-impregnated wheel before cementation. Cases delivered polished from the lab and re-polished after any adjustment show antagonist wear rates comparable to natural enamel-on-enamel contact.
Can a failed zirconia crown be repaired?
Chipping on zirconia is uncommon, and when it happens the crown almost always needs to be remade. Zirconia does not bond to composite the way silica-based ceramics do, so intraoral repair with flowable composite is a short-term fix, not a durable one. If the underlying prep is intact, a remake is straightforward from a stored digital design file. Every case we run stays in the design archive for at least seven years so a remake does not require a new impression.
How does zirconia compare to porcelain-fused-to-metal for longevity?
PFM crowns have a 20-year clinical track record and survive at rates comparable to monolithic zirconia, but the failure modes are different. PFM tends to chip at the porcelain-metal interface around year 10 to 15. Monolithic zirconia does not have that interface, so the chipping failure mode goes away. The tradeoff is esthetic, PFM with a well-built porcelain layer still outperforms zirconia in specific anterior situations. For posterior work, monolithic zirconia is the workflow-simpler and equally durable choice.
